The first day of practice is right around the corner. Please make sure all Final Forms are completed by both the Athlete and Parent/Guardian. There also must be a current Physical on file in order to be able to participate in the first official day of practice which is August 1st. Below is a link to Final Forms as well as if you need an updated physical form.
